Soccer Lessons Enrollment Agreement

This agreement (“Agreement”) is entered into between B&G Soccer and the Parent/Guardian for the enrollment of the child in soccer lessons.

 

1. Enrollment

1.1. By reading this Agreement, the Parent agrees to enroll the child in soccer lessons provided by the Program.

1.2. The Parent understands that the child will participate in age-appropriate soccer activities and training sessions tailored for children aged 2-5.

2. Fees and Payments

2.1. The Parent agrees to pay the fees for the soccer lessons as outlined in the fee schedule provided by the Program.

2.2. Payment is due prior to the start of each session or as otherwise agreed upon by both parties.

2.3. Failure to pay fees may result in the child’s exclusion from lessons until payment is received.

3. Attendance and Participation

3.1. The Parent agrees to ensure the child’s punctual attendance for each scheduled lesson.

3.2. The Parent understands that regular attendance is crucial for the child’s progress and development in soccer skills.

3.3. If the child is unable to attend a lesson due to illness or other reasons, the Parent agrees to notify the Program in advance.

4. Code of Conduct

4.1. The Parent agrees to adhere to the Program’s code of conduct, which includes but is not limited to:

  • Respecting coaches, staff, and other participants.
  • Encouraging positive behavior and sportsmanship in the child.
  • Not engaging in disruptive or inappropriate behavior during lessons.
5. Health and Safety

5.1. The Parent agrees to provide accurate information regarding the child’s health and any medical conditions that may affect participation in soccer lessons.

5.2. The Parent understands that participation in soccer activities involves inherent risks, and the Program shall not be liable for any injuries sustained during lessons, except in cases of gross negligence.

6. Media Release

6.1. The Parent grants the Program permission to use photographs, videos, or other media featuring the child for promotional or educational purposes related to the Program’s activities.

7. Termination of Agreement

7.1. Either party may terminate this Agreement with written notice to the other party.

7.2. In the event of termination, any outstanding fees shall be paid in full.

8. Governing Law

8.1. This Agreement shall be governed by and construed in accordance with the laws of New York.

9. Entire Agreement

9.1. This Agreement constitutes the entire understanding between the parties concerning the child’s enrollment in soccer lessons and supersedes all prior agreements and understandings.
